Cardiac Rehabilitation Service
A multi-disciplinary service that provides individualised holistic assessment of physical, psychological and social needs of patients and their families who have suffered a myocardial infarction (heart attack) or undergone cardiac surgery (stent implantation, bypass, and valve surgery)

The Cardiac Rehabilitation Service offers;
- A multi-disciplinary service that provides individualised holistic assessment of physical, psychological and social needs of patients and their families who have suffered a myocardial infarction (heart attack) or undergone cardiac surgery (stent implantation, bypass, and valve surgery)
- Education, advice and support on lifestyle and risk factor modification e.g. physical activity, diet, alchohol and smoking cessation
- Effective evidence based medication management in line with national and local guidance.
- Support with return to work
- Initial contact within five working days post discharge from hospital. This is provided by the Specialist Nurse for patients with a diagnosis of Myocardial Infarction (heart attack)
- Follow-up in clinic by Specialist Nurse for patients with a diagnosis of Myocardial Infarction (heart attack)
- Referral to the Cardiac Rehabilitation Exercise Programme in the closest locality to discuss Physical Activity
- 1:1 Psychological support is provided post MI/cardiac Surgery if required
- Functional/Daily activity needs are addressed
- Referral to stress and anxiety management programme
- Who can access the service?
This is an adult service for those over the age of 18 years.
The following patient groups are known to benefit and so should be offered a Cardiac Rehabilitation Programme irrespective of age, sex, ethnic group and clinical condition.
-acute coronary syndrome, including myocardial infarction and unstable angina
-coronary revascularisation
-post-heart valve repair/replacement
-post-heart transplantation and ventricular assist devices
-adult congenital heart disease (ACHD)
-spontaneous coronary artery dissection (SCAD)
Any patients that meet the above criteria and are willing to enrol into the Cardiac Rehabilitation Programme will be accepted.
All patients diagnosed with the above conditions will be reviewed by a member of cardiac rehabilitation multi-disciplinary team before being referred into the cardiac rehabilitation exercise programme as appropriate.
- Where are Cardiac Rehabilitation Exercise classes offered?
Exercise Classes are currently offered at Robin Park Leisure Centre and Leigh Leisure Centre.
- Where is the service offered?
The team will endeavour to offer you an appointment at the clinic closest to your home address.
We offer clinics across the Wrightington, Wigan and Leigh Teaching Hospitals NHS Foundation Trust area. These include Ashton, Aspull, Atherton, Boston House, Chandler House, Leigh, Parr Bridge, Pemberton, and Platt Bridge.
