We provide a nurse led comprehensive service for patients with type 2 diabetes supporting patients to achieve glycaemic control.
- About Us
We are a small team of Diabetes Specialist Nurses with the aim to provide high quality care for pateints diagnosed with type 2 diabetes. We support insulin initiation, titration and ongoing review.
We also provide education and advice for Priimary Care Health Care Professionals supporting patients living with Type 2 Diabetes.
We work collaboratively with the Diabetic Medicine Team and our Podiatry team.
- What is Type 2 Diabetes?
WHAT IS TYPE 2 DIABTETES?
- Type 2 diabetes is a common condition where your body cannot make enough of a hormone called insulin, or the insulin it makes does not work properly (insulin resistance).
- Insulin helps your body use glucose (sugar) for energy. Without enough insulin, the level of glucose in your blood becomes too high.
- Type 2 diabetes is different to type 1 diabetes, which is caused by a problem with your immune system. Unlike type 1 diabetes, type 2 diabetes may be prevented.
- You can manage type 2 diabetes with diet and exercise, or medicines.
- Having high blood glucose levels can cause complications such as heart disease, kidney disease, stroke or nerve problems.
- It may be possible to prevent type 2 diabetes or stop it (remission) by losing weight and improving your diet.

DESMOND stands for Diabetes Education and Self-Management for Ongoing and Newly Diagnosed.
More simply it is a way of finding out more about Type 2 diabetes, a resource to help you manage the changes diabetes will bring to your life, and an opportunity to meet and share experience with others.

How do I join DESMOND?
Self-Referral Information:

If you have a confirmed diagnosis of Type 2 diabetes, you can self-refer by calling the admin hub at 0300 707 7700 option 5
-
When you register, you can choose to attend:
-
Face-to-face sessions (2 half day sessions)
-
Online sessions - accessed at your own pace
-
Or both
We also accept referrals from Health Care Professionals
- Local Diabetes UK Support Groups
The Shevington Diabetes Support Group provides support and information to people living with diabetes, and to their family and friends.
We also hope to improve the understanding of diabetes by anyone with an interest in it and especially those who may be at risk of developing diabetes in the future.
We send out a warm welcome to anybody who would like to come along to one of our meetings, which are free to attend. We have excellent speakers and always have time for questions, refreshments and chats at the end of each meeting.
We believe that support, information and companionship are some of the simplest yet most important aspects of living with diabetes – this is why our group exists and we want to help in any way that we can.
Meeting 7pm first Monday of every month (except Bank Holidays) at Shevington Methodist Church, WN6 8HA.
