Introduction
The Community Neuro and Stroke team (CNST) provide a clinical pathway for individuals diagnosed with Functional Neurological Disorder (FND), alongside its usual service provision. This leaflet explains the FND pathway and outlines what you can expect when accessing support from CNST. It is intended to be read together with the main CNST service leaflet, which offers general information about the team and its wider services.
Referrals into The Service
Before being referred to our FND pathway, you will have received a confirmed diagnosis of FND from a Consultant Neurologist. Referrals can be made by your GP or other healthcare professionals.
All referrals are reviewed to make sure our service is the right place for you. As part of this process, you may have already taken part in a triage telephone call.
If your referral is accepted, you will be invited to attend our FND clinic with Dr Gaber, Consultant in Rehabilitation Medicine, along with an Occupational Therapist. Before this appointment, we will send you a pre‑clinic assessment and questionnaire. Please complete these and bring them with you. They will be collected around 10 minutes before your appointment, to allow the team to review them.
Your Clinic Appointment
Your appointment will usually last around 45 minutes. This is your opportunity to talk about your diagnosis, how your symptoms affect your day‑to‑day life and the areas where you would like support.
After Your Clinic Appointment
If it is appropriate and you consent, you will be added to our waiting list for treatment. Please note that waiting times can vary.
Our FND pathway is led by Occupational Therapy and focuses on helping you understand how your symptoms relate to your daily activities, roles, and responsibilities. Depending on your individual needs, you may also be referred to other members of the team, such as Physiotherapy or Speech and Language Therapy.
We may also suggest or refer you to other services that could support your well-being. This will only be done with your agreement.
Your treatment sessions may take place in your home, in an outpatient clinic or at another suitable location. Therapy will be tailored around your personal goals. We will work with you to explore what matters most to you and how therapy can help you make meaningful progress
