Intermediate Care Early Supported Discharge Facilitators

• About Us

  The aim of the Intermediate Care Early Supported Discharge Facilitators is to be aware of rehabilitation needs of the intermediate care patients admitted to Alexandra Court and Richmond House and to ensure that care and rehabilitation is transferred to community settings at the earliest opportunity.

  This will be achieved by tailoring and delivering the therapy based rehabilitation that the patients need around their own home environment, thus allowing earlier discharge from intermediate care.

  The service is open Monday to Friday, 8am – 5pm

• Who can access the service?

  Patients who are due for discharge from an intermediate care bed from Alexandra Court and Richmond House, who are medically stable, safe to remain in their home overnight without additional support and are able to consent and comply with interventions.
  Patients must be a Wigan Borough resident.
