OVERVIEW:
We are a small multidisciplinary team, who can assess and provide information and treatment programmes to promote self- management, for patients with long term swelling who meet our service criteria.
Patients aged 16 and over, who are registered with GP practices within the Wigan Borough / who are Wigan Borough residents, and who have long term swelling.
Any Healthcare Professional can refer into our service – see referral form below.
We are unable to accept self-referrals.
All referrals are triaged to assess eligibility, and those who meet our service criteria are offered an appointment for assessment.
Our clinics are based at Boston House, Frog Lane, Wigan.
Home Visits may also be available for housebound patients.
